There are many places where dreams come true. The operating room, too. Wishes are linked to positive situations, and regaining quality of life, when pain prevents normal life, is one of them for patients with pathologies. Before arriving there and meeting the surgical microscope in person, the consultation at Dr. Mosqueira Clinic served as a first contact to learn the diagnosis. Bernardo and patient forge their first emotional bond before writing a new story that will go down in history. Thus begins a day of surgery.

Preoperative progress: health ITV

Once the first stage is completed, the next step is the preoperative examination, which, as clinic coordinator José Morales explains, is usually performed a few days in advance, although there are exceptions, as is often the case. If the patient has a medical condition, it can be performed up to a week beforehand. What is the basis for this examination? It includes a questionnaire, a complete blood test, a chest X-ray, and an electrocardiogram.

Everything OK and we walk to the operating room

With the thumbs up and everything 'ok,' everything is ready for the day marked in green hope on the calendar: the day of the surgery. The patient arrives at the hospital, HM Hospitales Málaga, at the specified time. The data collection begins, and while waiting for the call to go up to the room, José Morales is always there. It's time to make the traditional "before the surgery" video, which all the clinic's followers will be able to see on social media. 

There, the patient is changed and the IV is taken. It should be noted that the patient has been fasting for at least 8 hours, since these procedures are performed under general anesthesia. From the room to the recovery room or Post-Anesthesia Recovery Unit (PACU). With the Anesthesia professionals supervising, this is where the entire procedure is detailed. The green light is given after the checklist and we transfer the patient to the operating room where, in less than an hour, he will achieve his precious and long-awaited quality of life. 

Patient blood pressure and heart rate monitoring. The anesthetic would have been the last thing on the radar, but before that step, the great Dr. Mosqueira arrives to reassure the patient, saying, "Everything's going to be fine." Now, general anesthesia is administered, the patient is positioned appropriately, and the operating room is prepared with all the equipment. Nurse Miguel Carlos prepares the instruments, accompanied by the assistant, orderly, anesthesiologist (already mentioned), and assistant. Everything is ready. 

We turn on the microscope…

Continuing with the day of surgery, Dr. Mosqueira must thoroughly wash his hands. This ensures maximum sterilization so that the surgical area and the surrounding environment are free of microorganisms, thus preventing infections (asepsis). "Now we puncture the area to be treated and, using the X-ray machine, mark the location down to the millimeter. This allows us to act on the root cause of the problem using the surgical microscope. This makes the incision minimal, just 3-4 centimeters." 

This is where the surgical microscope, guided by the eyes and hands of neurosurgeon Bernardo, comes into play. The techniques: microlaminoforaminotomy for spinal canal stenosis and microdiscectomy for herniated discs, both lumbar and cervical. Opening, cleaning, decompression, and suturing.
